Service Coordinators

Targeted Case Management

Service Coordinators (SC) develop and maintain relationships with community groups and agencies in order to educate clients about community services. They work closely with Therapists, Medical Providers, Community Support Associates and Peer Support Specialists to assist clients with independence and growth.

Assistance with...

  • Applying for Medicaid or  changing your MCO
  • Applying for food stamps  and helping with re-certification  of food stamp case.
  • Linking to a dentist, eye doctor, family doctors (PCP), specialist, and linking to other providers to create a team based holistic approach.
  • Create a treatment team within A.I.M. Wellness and facilitate communication between providers. We will work with medication management, PCP, CSA, PSS, IOP, ABA, and OT to ensure the best quality treatment.
  • Work within the school system to facilitate 504/IEP meetings, and communicate with counselors and teachers to ensure your child is successful in school.
  • Admittance to psychiatric inpatient facility as needed
  • Affording medications
  • Obtaining drivers license, social security card, and birth certificates
  • Navigating DCBS and state programs
